浸润性乳腺癌中Claudin 18.2和MVD的相关性及与临床病理特征的关系

Correlation between Claudin 18.2 and MVD in invasive breast cancer and its relationship with clinicopathological features

  • 摘要: 目的 探讨浸润性乳腺癌中紧密连接蛋白18.2(Claudin 18.2)和平均微血管密度(MVD)的表达及与临床病理参数的关系,分析两者之间的相关性,评估其在乳腺癌诊断及预后评估中的潜在应用价值。方法 选取郑州人民医院病理科2017年1月至2018年6月期间存档的78例浸润性乳腺癌组织蜡块,选取同期保存的乳腺良性病变组织27例,采用免疫组织化学SP法检测Claudin 18.2与CD34的表达。用ImagePro Plus(IPP)图像分析软件对Claudin 18.2蛋白表达进行量化分析,通过计数5个高倍视野下的CD34阳性血管数量,计算其平均值以代表MVD。分析Claudin 18.2与MVD的表达水平与患者年龄、部位和WHO分级等临床病理特征的关系,用Spearman相关法分析Claudin 18.2与MVD的相关性。结果 Claudin 18.2主要定位于浸润性乳腺癌的细胞膜上,CD34则主要标记位于癌巢周围间质的血管内。浸润性乳腺癌组织中Claudin 18.2蛋白表达量与MVD的数目较乳腺良性病变组织均显著上调(P=0.000)。Claudin 18.2的蛋白表达量在WHO III级(12.49±5.97)中显著高于I-II级(10.26±3.94),且在有淋巴结转移的病例中(12.65±5.91)高于无淋巴结转移者(10.22±4.00)(P< 0.05)。MVD值在WHO III级(48.88±49.40)中高于I-II级(32.05±21.36),在孕激素受体(PR)低表达组(43.61±44.30)中高于高表达组(32.72±21.17),且在HER2扩增组(50.82±51.18)中显著高于非扩增组(34.82±28.90)(P< 0.05)。Spearman相关分析表明,浸润性乳腺癌组织中Claudin 18.2和MVD呈显著正相关(r=0.973,P=0.000)。结论 浸润性乳腺癌中Claudin 18.2与MVD的高表达与肿瘤的发生发展密切相关,且两者间存在显著的正相关性。检测Claudin 18.2蛋白的定量表达及MVD对于评估乳腺癌的侵袭与转移具有重要临床意义,可为浸润性乳腺癌的病理诊断、个体化治疗及预后评估提供一定参考依据。

     

    Abstract: Objective This study aimed to investigate the expression of Claudin 18.2 and mean vascular density (MVD) in invasive breast cancer (IBC) and their correlation with clinicopathological parameters. Furthermore,we analyzed the relationship between Claudin 18.2 and MVD,and assess their potential application value in breast cancer diagnosis and prognosis evaluation. Methods A total of 78 paraffin-embedded IBC tissue blocks achieved from the Pathology Department of Zhengzhou People’s Hospital from January 2017 to June 2018 were selected,and 27 benign breast lesion tissue samples were selected from the same period. The expression of Claudin 18.2 and CD34 were detected by immunohistochemical SP method. The expression of Claudin 18.2 protein was quantitatively analyzed using ImagePro Plus (IPP) image analysis software,while the number of CD34-positive vessels in five high-power fields were counted with their average was calculated to represent MVD. The relationships between the expression levels of Claudin 18.2 and MVD with clinicopathological features such as age,location,and WHO grade were analyzed. Spearman’s correlation analysis was performed to examine the correlation between Claudin 18.2 and MVD. Results Claudin 18.2 was mainly localized to the cell membrane of IBC,while CD34 mainly labeled blood vessels within the stroma surrounding cancer nests. The expression of Claudin 18.2 protein and MVD in IBC tissues were significantly upregulated compared with those in benign breast lesion tissues (P=0.000). The expression of Claudin 18.2 protein was significantly higher in WHO grade III (12.49±5.97) than in grades I-II (10.26±3.94),and higher in cases with lymph node metastasis (12.65±5.91) than those without (10.22±4.00) (P< 0.05 for both). The MVD value was higher in WHO grade III (48.88±49.40) than in grades I-II (32.05±21.36),higher in the low progesterone receptor (PR) expression group (43.61±44.30) than in the high expression group (32.72±21.17),and significantly higher in the HER2 amplified group (50.82±51.18) than in the non-amplified group (34.82±28.90) (P< 0.05 for all). Spearman correlation analysis revealed a significant positive correlation between Claudin 18.2 and MVD in IBC tissues (r=0.973,P=0.000). Conclusion The high expression of Claudin 18.2 and MVD in IBC is closely related to the occurrence and development of tumors,and there is a significant positive correlation between them. The quantitative detection of Claudin 18.2 protein expression and MVD are of great clinical significance for assessing breast cancer invasion and metastasis,providing a reference for pathological diagnosis,individualized treatment,and prognosis evaluation of IBC.
